To port your phone number, please complete these steps:
1. Download and Complete the LOA:Download and complete the attached
Letter of Authorization (LOA). Ensure you include your service account number and that your service address, billing account number, and billing telephone number match your current carrier's records.
2. Gather Required Documents:- Your completed LOA.
- A copy of your billing statement issued within the last 30 days.
3. Prepare Your PIN:If you are porting a wireless/mobile number, you will likely need your 4-digit (or longer) account PIN. You can find this on your Customer Service Record (CSR) or request it from your current carrier.
4. Upload Below:Please upload both your completed LOA and your billing statement.
